	An “official preliminary to the 2009 MISS TEXAS USA® pageant” (to be held summer of 2008). Independently Produced By Diamond Productions. I hereby apply as an entrant in the MISS DALLAS USA, and I acknowledge and agree as follows:
1.	I will be at least eighteen (18) and under twenty-seven (27) years of age on February 1, 2009.
2.	I am not married, have never been married and have never had a marriage annulled. I  have  never given birth to a child and am not now pregnant nor am I a parent. If any of these conditions change, I will notify my Local Director immediately, and I understand that this may affect my eligibility.
3.	I have never participated in a previous national MISS USA® pageant.
4.	I have not and will not participate in any other state pageant (including District of Columbia) preliminary to the 2009 MISS USA® pageant.
5.	I am of good health and moral character.
6.	I represent that I have not and will not engage in any activities which, if disclosed to the public, would bring, or tend to bring, myself, my local pageant, the MISS TEXAS USA® pageant, The Crystal Group, Miss Universe L.P., LLLP, Trump Pageants, Inc., The Trump Organization, NBC, Inc. or any of their subsidiaries, affiliated or related companies into disrepute, ridicule or contempt not limited to, but to include any type of use of illegal drugs, any other illegal behavior or activity, professional or amateur nude or semi-nude dancing or modeling in any sexually oriented business or club, any type of nude or semi-nude photographs or video tapes, prostitution, any felony conviction, probation or misdemeanor involving moral turpitude.
7.	I am a naturally-born female and am a citizen of the United States and at least one of the following is true: (a) I now reside and have resided in my Texas city as my permanent and primary residence for a period of at least six (6) months immediately prior to the beginning of my local pageant (although I may have attended college or university elsewhere); or (b) I have resided in my Texas city as a full-time student at college or university for one entire semester immediately prior to the beginning of my local pageant, and I am continuing to reside in my local area as a full-time student (although I may have spent or be spending the summer in another city or state).
8.	I agree that if selected to participate in my state MISS TEXAS USA® pageant I will participate in that pageant and will abide by all rules and regulations governing that pageant. If I win the MISS TEXAS USA® pageant, then I agree to participate in the MISS USA® pageant and abide by the rules and regulations governing that pageant. If I win the MISS USA® pageant, then I agree to participate in the MISS UNIVERSE® pageant and abide by the rules and regulations governing that pageant.
9.	Neither I nor any member of my immediate family is employed by Miss Universe L.P., LLLP, any State Director in the MISS USA® and/or MISS TEEN USA® pageant system, NBC, Inc. or any of its subsidiaries, Ernst & Young, or any of its subsidiaries, Donald J. Trump, Trump Pageants, Inc., The Trump Organization, NBC Pageants, Inc. or any of their subsidiaries or affiliated or related companies. (IF YOU OR ANY MEMBER OF YOUR IMMEDIATE FAMILY IS EMPLOYED BY ANY OF THE FOREGOING OR ANY FIRM OR CORPORATION CONTROLLED OR AFFILIATED WITH ANY OF THEM, THEN YOU MUST NOTIFY YOUR LOCAL DIRECTOR IMMEDIATELY IN WRITING, SO THAT YOUR ELIGIBILITY MAY BE DETERMINED.)
10.	I agree that if I win or succeed to the state level title or the title of MISS USA®, then I will not participate in any pageants during the time that I hold either of those titles, unless authorized in writing by The Crystal Group, Inc. or by Miss Universe L.P., LLLP.
11.	I HEREBY RELEASE, UNCONDITIONALLY AND FOREVER ANY CLAIM AGAINST MY LOCAL PAGEANT, DIAMOND PRODUCTIONS, THE CRYSTAL GROUP, INC., AND/OR MISS UNIVERSE L.P., LLLP, THEIR PARENT OR AFFILIATED OR RELATED COMPANIES AND THEIR RESPECTIVE OFFICERS, DIRECTORS, PARTNERS, EMPLOYEES, AGENTS AND ASSIGNS, WHICH I MAY HAVE BY VIRTUE OF MY PARTICIPATION IN MY LOCAL AND STATE PAGEANT OR BY ANY USE OF MY NAME, LIKENESS, VOICE AND/OR BIOGRAPHY IN CONNECTION WITH MY LOCAL AND STATE PAGEANT, INCLUDING USE IN PROMOTIONAL AND ADVERTISING MATERIAL.
12.	I understand and agree that all issues as to eligibility shall be determined by my local director, The Crystal Group, Inc. or Miss Universe L.P., LLLP, and I agree to abide by that determination both as to myself and other applicants.
